Trailblazers Training Together

This project, funded by Bristol, North Somerset and South Gloucestershire Clinical Commissioning Group, Ageing Well Service Development Fund, aims to support hard to reach groups and individuals through community volunteers.  Volunteers will help to introduce technology and support increased independent at the end of the reablement programme, lasting 6 weeks.

Aims of the project:

For patients

  • to increase levels of physical activity, leading to better physical and mental health outcomes
  • improved digital literacy
  • to achieve greater independence
  • to reduce social isolation
  • to increase patients confidence moving between primary and secondary care services

For Health and Social volunteers

  • to provide confidence that patients from all backgrounds can access digital technology for clinical interactions, self manage conditions and increase their physical activity and social connectivity
  • increase their knowledge, skills and tools to have conversations about physical activity and to signpost patients to opportunities for independent activity at home and in the community
